While it is recommended that players enter as a team, you may enter individually and
will be placed on a team. Complete the form ENTIRELY and return it to Three J’s
Cigars with payment. Phone entries only accepted with credit card payment.
Acceptable payment by Cash, Check or Credit Card. Checks should be made
payable to Three J’s Cigars and in the memo line note: Charity Golf Extravaganza. Credit Card payments MUST be made either at Three J’s Cigar Shop or by calling the store at 561-868-5600.

By a “Blind Draw,” one of the teams playing in the tournament will represent your charity. If your team wins, your charity will receive $1,500.00 of the event proceeds. All sponsor’s get a hole sign on the golf course and the opportunity to put a tangible promotional item in every player’s gift bag.
